Position Summary

The CFSS Support Worker provides person-centered assistance with activities of daily living (ADLs), instrumental activities of daily living (IADLs), health-related procedures and tasks, and observation and redirection of behaviors as identified in the participant’s CFSS service delivery plan. The Support Worker delivers services in the participant’s home or community setting with a focus on dignity, safety, and individual choice. All services are performed within the scope of the participant’s authorized plan and agency policy.

Essential Duties and Responsibilities

  • Provides assistance with ADLs as identified in the service delivery plan, which may include bathing, grooming, dressing, toileting, eating, mobility, positioning, and transfers.
  • Provides assistance with IADLs as identified in the plan, such as meal preparation, light housekeeping essential to the participant’s health, laundry, and shopping.
  • Performs health-related procedures and tasks ONLY as identified in the service delivery plan and only after the agency has verified worker competency for the specific task; assistance with self-administered medications as authorized in the plan.
  • Provides observation and redirection of behaviors as identified in the plan.
  • Observes and promptly reports to the agency changes in the participant’s condition, behavior, or home safety; reports any request to perform a task not in the service delivery plan.
  • Completes EVV check-in/check-out for every visit and accurate time-and-activity documentation supporting the units billed to Medicaid (MN-ITS).
  • Participates in supervision and performance evaluation activities, including required worker evaluation visits.
  • Performs other duties only as authorized within the service delivery plan and agency policy.

Compliance and Regulatory Responsibilities

  • Maintains participant confidentiality and complies with HIPAA; no photography of participants or sharing of participant information, including on social media.
  • Serves as a mandated reporter of suspected maltreatment of vulnerable adults (Minn. Stat. 626.557) and minors (Minn. Stat. ch. 260E).
  • Honors participant rights, dignity, cultural preferences, and person-centered service principles under CFSS program requirements.
  • Follows basic infection control practices, including hand hygiene and safe food handling; uses PPE as directed.
  • Accepts no gifts, loans, or financial arrangements with participants; follows agency client-funds policy for any authorized shopping.
  • Understands that falsified time records or billing for services not provided constitutes Medicaid fraud subject to termination, repayment, and criminal referral.

Scope of Practice

  • Performs ONLY tasks identified in the participant’s CFSS service delivery plan for which competency has been verified.
  • Does NOT perform sterile procedures, injections, or nursing tasks requiring a license; does NOT administer medications (assistance with self-administration only as authorized).
  • Does NOT make clinical judgments, change the service delivery plan, or provide services to participants not assigned by the agency.
  • Does NOT transport participants in a personal vehicle unless specifically authorized in writing by the agency with verified license and insurance.

Required Qualifications

  • Meets minimum age requirement for CFSS support workers under Minn. Stat. 256B.85.
  • Cleared Minnesota DHS background study (Minn. Stat. ch. 245C, NETStudy 2.0) prior to direct contact with participants.
  • Successful completion of all DHS-required support worker training and agency orientation BEFORE providing services.
  • Demonstrated competency, verified by the agency, to perform each service and task listed in the participant’s service delivery plan before performing it.
  • Ability to effectively communicate with participants and representatives sufficient to provide services safely.
  • Tuberculosis screening consistent with MDH/CDC requirements prior to participant contact.
  • Reliable transportation to assigned participant locations; valid driver’s license and auto insurance required when driving is part of the authorized assignment.
  • Annual in-service training as required by DHS and agency policy.

Physical Requirements

  • Frequent travel between participant homes; ability to operate a motor vehicle safely.
  • Ability to lift, carry, push, or pull up to 50 pounds occasionally and up to 25 pounds frequently, consistent with safe client handling policies.
  • Frequent standing, walking, bending, stooping, kneeling, and reaching; assisting clients with mobility and transfers where within role scope.
  • Sufficient visual, hearing, and communication ability to observe clients, respond to calls for help, and communicate verbally and in writing.
  • Manual dexterity sufficient to perform assigned tasks and to document in the electronic record.
  • Potential exposure to communicable disease, bloodborne pathogens, household pets, variable home environments, and adverse weather. PPE provided per agency policy.

Schedule

Alliance Home Care offers full-time, part-time, and PRN opportunities. Visit schedules and shift expectations vary by case assignment and will be reviewed during the interview process.
