Position Summary

The Home Health Aide provides personal care and aide services to clients in their homes under a written aide assignment prepared by the Registered Nurse as part of the plan of care, in accordance with 42 CFR 484.80 and Minnesota home care rules. The HHA performs assigned personal care tasks, observes and reports client status changes to the supervising RN, and supports the client’s comfort, safety, and independence at home.

Essential Duties and Responsibilities

  • Provides hands-on personal care as listed on the written aide assignment: bathing, grooming, dressing, oral care, skin care, toileting and elimination assistance, and feeding assistance.
  • Provides safe transfer techniques, ambulation assistance, and positioning as assigned.
  • Performs simple procedures as an extension of therapy or nursing services ONLY when specifically assigned and after competency verification (e.g., assistance with prescribed range-of-motion exercises).
  • Assists with self-administered medications only as assigned; does NOT administer medications.
  • Performs light household services essential to the client’s health care at home (e.g., changing bed linens, meal preparation) when assigned.
  • Takes and records vital signs only when assigned and competency-verified.
  • Observes, records, and promptly reports to the supervising RN changes in the client’s condition, skin integrity, behavior, appetite, or home safety; reports any client/family request for tasks not on the aide assignment.
  • Completes a visit record for every visit documenting tasks performed, arrival/departure times, and observations; uses EVV where required.
  • Participates in supervisory visits, case conferences, and required in-services.

Compliance and Regulatory Responsibilities

  • Maintains confidentiality and complies with HIPAA and agency privacy/security policies in all settings.
  • Complies with infection prevention and control policies consistent with 42 CFR 484.70 and OSHA Bloodborne Pathogens standard; uses standard precautions and PPE.
  • Serves as a mandated reporter of suspected maltreatment of vulnerable adults (Minn. Stat. 626.557) and minors (Minn. Stat. ch. 260E).
  • Honors client rights under 42 CFR 484.50 and the Minnesota Home Care Bill of Rights (Minn. Stat. 144A.44).
  • Completes orientation and annual in-service/continuing education required by agency policy and Minnesota home care rules.

Scope of Practice

  • Performs ONLY tasks on the written, client-specific aide assignment prepared by the RN; if a task is not on the assignment, the HHA declines and calls the office.
  • Does NOT administer medications, perform sterile procedures, perform wound care beyond assigned simple tasks, insert/remove tubes or devices, or perform any task requiring a nursing or therapy license.
  • Does NOT make clinical judgments or assessments; observations are reported to the RN.

Required Qualifications

  • Successful completion of a qualifying home health aide training program (minimum 75 hours, including at least 16 hours of supervised practical training) per 42 CFR 484.80(a).
  • Competency evaluation successfully completed before performing client care, covering all required subject areas of 42 CFR 484.80(b)–(c).
  • Cleared Minnesota DHS background study (Minn. Stat. ch. 245C) prior to direct contact with clients.
  • Current CPR certification per agency policy.
  • Valid driver’s license, acceptable driving record, current auto insurance, and reliable transportation.
  • Completion of at least 12 hours of in-service training during each 12-month period (42 CFR 484.80(e)).
  • Ability to read, write, and follow the written aide assignment and to communicate observations effectively.

Physical Requirements

  • Frequent travel between client homes; ability to operate a motor vehicle safely.
  • Ability to lift, carry, push, or pull up to 50 pounds occasionally and up to 25 pounds frequently, consistent with safe client handling policies.
  • Frequent standing, walking, bending, stooping, kneeling, and reaching; assisting clients with mobility and transfers.
  • Sufficient visual, hearing, and communication ability to observe clients, respond to calls for help, and communicate verbally and in writing.
  • Manual dexterity sufficient to perform assigned tasks and to document in the electronic record.
  • Potential exposure to communicable disease, bloodborne pathogens, variable home environments, and adverse weather. PPE provided per agency policy.

Schedule

Alliance Home Care offers full-time, part-time, and PRN opportunities. Visit schedules and shift expectations vary by case assignment and will be reviewed during the interview process.
