Position Summary

The Occupational Therapist evaluates and treats clients in their homes under physician/allowed practitioner orders and the plan of care, in accordance with Minnesota’s Occupational Therapy Practice Act (Minn. Stat. ch. 148.6401–.6450) and Medicare Conditions of Participation (42 CFR 484). The OT is responsible for occupational therapy evaluation, skilled intervention, COTA supervision, reassessment, and discharge planning for assigned cases.

Essential Duties and Responsibilities

  • Performs occupational therapy evaluations and reevaluations; establishes measurable, client-centered goals and the OT component of the plan of care under physician/allowed practitioner orders.
  • Completes the comprehensive assessment, including OASIS, on qualifying cases consistent with 42 CFR 484.55.
  • Provides skilled OT interventions, including ADL/IADL retraining, neuromuscular re-education, energy conservation, cognitive and perceptual training, adaptive equipment assessment and training, and home safety evaluation.
  • Performs the qualified-therapist functional reassessment at least every 30 calendar days while OT continues.
  • Delegates selected interventions to Occupational Therapy Assistants per Minn. Stat. 148.6432; conducts face-to-face collaboration with each COTA as required; does not delegate evaluation, reevaluation, or plan-of-care functions.
  • Conducts home health aide supervisory visits on designated therapy-only cases per 42 CFR 484.80(h).
  • Documents homebound status and medical necessity at each visit; coordinates equipment orders; participates in case conferences and QAPI.

Compliance and Regulatory Responsibilities

  • Maintains confidentiality and complies with HIPAA and agency privacy/security policies in all settings.
  • Complies with infection prevention and control policies consistent with 42 CFR 484.70 and OSHA Bloodborne Pathogens standard.
  • Serves as a mandated reporter of suspected maltreatment of vulnerable adults (Minn. Stat. 626.557) and minors (Minn. Stat. ch. 260E).
  • Honors client rights under 42 CFR 484.50 and the Minnesota Home Care Bill of Rights (Minn. Stat. 144A.44).

Required Qualifications

  • Graduate of an ACOTE-accredited occupational therapy program (or equivalent qualification pathway meeting 42 CFR 484.115).
  • Current, unencumbered license as an Occupational Therapist issued by the Minnesota Board of Occupational Therapy Practice.
  • Current NBCOT certification (OTR) or successful completion of the NBCOT examination.
  • Current CPR/BLS certification.
  • Valid driver’s license, acceptable driving record, current auto insurance, and reliable transportation.
  • Cleared Minnesota DHS background study (Minn. Stat. ch. 245C) prior to direct contact with clients.

Preferred Qualifications

  • 1+ years of clinical experience; home health and OASIS experience strongly preferred.
  • Experience supervising Occupational Therapy Assistants.
  • Competency in electronic documentation (KanTime EMR or similar).

Physical Requirements

  • Frequent travel between client homes; ability to operate a motor vehicle safely.
  • Ability to lift, carry, push, or pull up to 50 pounds occasionally and up to 25 pounds frequently.
  • Frequent standing, walking, bending, stooping, kneeling, and reaching; assisting clients with mobility.
  • Sufficient visual, hearing, and communication ability to observe clients and communicate verbally and in writing.
  • Potential exposure to communicable disease, bloodborne pathogens, variable home environments, and adverse weather. PPE provided per agency policy.

Schedule

Alliance Home Care offers full-time, part-time, and PRN opportunities. Visit schedules and shift expectations vary by case assignment and will be reviewed during the interview process.
