Position Summary

The Registered Nurse delivers skilled nursing care to clients in their homes and community settings, in accordance with physician-ordered plans of care, Minnesota RN scope of practice (Minn. Stat. ch. 148), Minnesota home care statutes (Minn. Stat. ch. 144A), and Medicare Conditions of Participation (42 CFR 484). The RN is responsible for completing OASIS assessments, developing and managing the plan of care, providing skilled interventions, supervising LPNs and HHAs on assigned cases, educating clients and families, and coordinating care across the interdisciplinary team. This role is ideal for a self-directed clinician who thrives in an autonomous, community-based environment and is committed to helping clients achieve their goals at home.

Essential Duties and Responsibilities

Assessment and Plan of Care

  • Completes the initial assessment visit, comprehensive assessment, and OASIS data collection within required timeframes (42 CFR 484.55); ensures OASIS accuracy and locks documentation per agency policy.
  • Develops, implements, and updates the individualized plan of care in collaboration with the physician/allowed practitioner, client, family, and interdisciplinary team.
  • Performs ongoing clinical reassessments and OASIS recertification and discharge assessments as required by Medicare and agency policy.
  • Identifies changes in client condition promptly and adjusts the plan of care, physician orders, and care team communications accordingly.

Skilled Nursing Care

  • Performs skilled nursing visits in accordance with the physician-ordered plan of care and within the Minnesota RN scope of practice.
  • Administers medications, including IV therapy, IM/SQ injections, and complex medication regimens; monitors for therapeutic response and adverse effects.
  • Performs wound care, ostomy care, catheter care, tracheostomy care, and other skilled nursing procedures as ordered.
  • Conducts clinical observations and assessments including vital signs, pain evaluation, cardiovascular and respiratory monitoring, and functional status.
  • Provides compassionate, person-centered care that respects client dignity, preferences, cultural background, and goals.

Client and Family Education

  • Develops and delivers individualized education plans for clients and family caregivers covering disease management, medication regimens, treatment procedures, and home safety.
  • Teaches clients to recognize and respond to signs and symptoms requiring prompt reporting or emergency intervention.
  • Supports clients in building the skills and confidence for self-management of chronic conditions and care needs.

Supervision of LPNs and Home Health Aides

  • Provides clinical supervision and direction to LPNs working on assigned cases; reviews LPN assessments and clinical decisions and provides guidance within Minnesota supervisory requirements.
  • Prepares written HHA assignments for assigned cases; conducts supervisory visits to client homes at least every 14 days (42 CFR 484.80(h)); conducts annual direct-observation evaluations of HHA performance.
  • Delegates tasks to LPNs and HHAs consistent with their respective scopes of practice and competency; remains accountable for delegated care.

Care Coordination and Communication

  • Communicates significant changes in client condition, safety concerns, and newly identified needs to the Director of Nursing, physician, and relevant care team members promptly.
  • Collaborates with physical, occupational, and speech therapists, social workers, the intake team, and community partners to provide coordinated, seamless care.
  • Participates in case conferences, team meetings, and handoff communications as scheduled.
  • Coordinates with the scheduling team to manage visit cadence, respond to urgent client needs, and ensure continuity of care.

Documentation and Compliance

  • Documents all visits, assessments, interventions, client responses, and communications accurately and on time in the electronic health record (KanTime EMR) per agency and regulatory requirements.
  • Ensures documentation supports homebound status, medical necessity, skilled need, and all billed services for Medicare and Medicaid claims.
  • Maintains compliance with CMS Conditions of Participation (42 CFR 484), Minnesota home care statutes, agency policies, HIPAA, and the Minnesota Home Care Bill of Rights (Minn. Stat. 144A.44).
  • Serves as a mandated reporter of suspected maltreatment of vulnerable adults (Minn. Stat. 626.557) and minors (Minn. Stat. ch. 260E).
  • Supports agency readiness for MDH home care licensing surveys and CMS certification audits.

Required Qualifications

  • Current, unencumbered Registered Nurse (RN) license in the State of Minnesota.
  • Current Basic Life Support (BLS) / CPR certification.
  • Valid driver’s license and reliable personal vehicle with current auto insurance.
  • Ability to pass a Minnesota DHS background study (Minn. Stat. ch. 245C).
  • Strong clinical judgment, autonomous decision-making, and time management skills.
  • Effective written and verbal communication skills.

Preferred Qualifications

  • 1+ years of RN experience in home health, community health, medical-surgical, or a post-acute care setting.
  • OASIS experience or OASIS certification (COS-C).
  • Experience managing complex chronic conditions (CHF, COPD, diabetes, wound care, IV therapy) in a home or community setting.
  • Familiarity with Minnesota home care regulations, the Home Care Bill of Rights, and PDGM.
  • Experience with KanTime EMR or comparable home health documentation system.

Required Training (Provided or Verified by Alliance Home Care)

  • Agency orientation, including the Minnesota Home Care Bill of Rights, client rights, and agency policies prior to providing client care.
  • Vulnerable Adults Act and maltreatment mandated reporter training.
  • Infection control, standard precautions, and HIPAA training.
  • OASIS competency training prior to completing assessments independently.
  • Annual competency updates as required by agency policy and Minnesota home care regulations.
  • Ongoing continuing education to maintain active RN licensure.

Physical Requirements

  • Ability to perform clinical procedures in varied home environments, including bending, kneeling, and working in confined spaces.
  • Ability to lift and assist clients as needed, up to 50 lbs (approximately 22 kg).
  • Manual dexterity for medication administration, wound care, IV therapy, and EHR documentation.
  • Ability to drive to multiple client locations throughout the day in varying weather conditions.
  • Ability to respond appropriately during client emergencies.
