SKILLED NURSING

Skilled nurse visits are intermittent nursing services ordered by a physician for a recipient whose illness, injury, physical, or mental condition creates a need for the service. Services under the direction of a registered nurse (RN) are provided in the recipient’s residence by an RN or a licensed practical nurse (LPN), and provided under a plan of care or service plan that specifies a level of care that the nurse is qualified to provide.

COVERED SERVICES

SNVs include any of the following tasks:

    • Completion of a procedure requiring substantial and specialized nursing skill (e.g., administering intravenous therapy, intra-muscular injections and sterile procedures)
    • Consumer teaching and education/training that requires a nurse’s skills
    • Observation, assessment and evaluation of a person’s physical and/or mental status

SNVs include any of the following tasks:

    • Based on an assessed need
    • Delivered as described and documented in the person’s care or service plan
    • Made in accordance with the Minnesota Nurse Practice Act
    • Medically necessary
    • Ordered by a physician.
